"New Patient" Visit | $150
The Initia New Patient visit is a complex detailed visit as we obtain pertinent health history, family history, current medication list, and obtain previous medical records (if necessary). These visits typically take about 30 minutes, and can include treatment for wide variety of health issues.
Follow-Up Visit | $75
A Follow-Up Visit typically takes about 15 minutes and can be a follow-up to chronic diseases management or include treatment for one of the most common health issues listed below!

Phone Consultation for Established Patients | $40
Established patients seeking additional access to their doctors beyond face-to-face visits can schedule phone consultations for a small fee. Rate is $40 per 10 minutes.
Comprehensive Physical or Pre-Operative Exam | $200 - $300
LABORATORY TESTS INCLUDED!
CBC, CMP, Lipid Panel, HgB A1C
Sports or School Physicals (13+) | $35
Standard physical examinations to ensure that the individual is healthy and physically prepared to perform in camp, school, or sports. Specific requirements may vary by school, camp or sports association
